Specimen Required: | Collect: Buccal swab (#2 swabs) OR Sample Type B – whole blood collected in Lavender-top (EDTA) tube. Special Instructions: Please provide 3 MANDATORY forms: Informed consent and self-pay form; requisition form. Specimen Preparation: Buccal Swab : •Puritan™ PurFlock™ Ultra Flocked Swabs (Fisher Scientific, Cat. No. 22-25-192) •Puritan™ HydraFlock™ Swabs, standard tip (Puritan, Cat. No.25-3306-H) •Sterile Foam Tipped Swabs (Puritan, Cat. No.25-15061PF) •4N6FLOQSwabs™,regular tip (ThermoFisher, Cat. No.4473979) Note: Use of cotton or generic polyester swabs may result in lower DNA yields or DNA that contains PCR inhibitors. The proper swabbing procedure is critical to avoid inclusion of PCR inhibitors and to ensure sufficient DNA yields. Wear gloves when handling swabs •For best results, direct test subjects to thoroughly rinse their mouths with water prior to swabbing •Carefully remove the swab from its pouch Note: Do not fully separate the two sides of the pouch •Vigorously rub the inside of each cheek and over the gums of the test subject for 30 seconds (1minute total per swab) •Collect one swab at a time, and hold the swab as close to its head as is comfortable for the test subject •Air dry the swab for 1 minute, then carefully place it back into its original pouch •Two swabs from the same test subject may be placed in the same pouch •Place swab samples in their original pouches in to a secondary envelope for shipping •For al samples, record the collection date and storage temperature before shipping Store buccal swabs in the original pouch. Do not use plastic tubes. Bacterial growth in sealed plastic tubes can cause DNA degradation. Storage/Transport Temperature: Refrigerated (2-8°C). Stability: Up to 5 days at 2-8°CRefrigerated. Specimen Rejection Criteria: 1.Any sample that is unlabeled/mislabeled. 2.Any sample without a test requisition. 3.Buccal swabs in any preservative or media. 4.Any swab not in its original pouch. 5. Any duplicate test orders for previously tested patients. Whole blood collected in Lavender-top (EDTA): Collect whole blood specimens in EDTA tube Minimum of 2-4 mL using acceptable phlebotomy techniques. Invert the tubes 2-3 times after collection to ensure anticoagulation. Samples should be kept in original tube, not frozen, and not hemolyzed. Storage/Transport Temperature: Room Temperature Stability: Room Temperature (preferred) or Refrigerated (2-8 °C) for a maximum of 5 days after collection. Specimen Rejection Criteria: •Any sample that is unlabeled/mislabeled. •Any sample without a test requisition. •Any specimen in an unsuitable container. (heparin etc.). •Frozen/hemolyzed specimens. •Specimens with insufficient quantity.
